Final Forms 

In order to participate in athletics at Gull Lake High School or Middle School students MUST have an online Final Forms account.  This is where students can sign up for sports they are interested in and complete all necessary forms.  To access or create your account follow the link below.  Once a student has an account created they will use this same account for the remainder of their time at Gull Lake CS.

• Participation Fee: $50/sport for MS and $75/sport for HS
  (due at the start of each season)

• Pay online at Efunds

• A current athletic physical is required.

  • Valid Physical Must be dated after 4/15/2024

• Sports Physical Day: Thursday, August 8, 2-5pm at GLHS
